This is far from being a definitive list, but summarizes the average length
and quality of most of the JA tapes in circulation.  In the case of TV
broadcasts, the quality is for the audio only.  
Sources are as follows:
  AU: Recorded from the audience
  FM: FM broadcast
  SB: Soundboard recording
  ST: Studio recording
  TV: TV broadcast (usually mono)
  VT: Video tape  
Quality is the average of tapes circulating for each date.  Obviously
someone must have high-quality masters, and there could be several source
audience recordings for some shows, so this is just a general grade.
Quality grades are P: poor, G: good, VG: very good, EX: excellent  
Rarity: Range from 1 to 5 (1 = "everyone" has it, 5 = very hard to find)

VIII. Commonly Mislabeled Tapes  
Most of these are mentioned in the set lists, but are summarized in
this section.  
Oct 16 1965 - Longshoreman's Hall, San Francisco CA
  Although this show did take place on this date, tapes claiming to
  be this show are most likely edited from one of the Fillmore shows from
  Oct 14, 15 or 16 1966.  Fat Angel in Oct 1965??  Donovan didn't record
  it until May 1966!  
Jun 22, 23 1966 - Avalon Ballroom, San Francisco CA
  Most Avalon shows are well-documented, and there is no evidence that
  JA played the Avalon on these dates.  They did play the Avalon on
  July 22 and 23, and it's likely that tapes with these dates may be
  either of these shows, although July 22 is the most likely date since
  tapes of July 23 are not known to be in circulation.  
Oct 5 1966 - Winterland, San Francisco CA
  Bill Graham's concerts are very well documented, and there is no proof
  that JA played the Winterland on this date.  Since I have not seen this
  tape, I don't know what it really is, although I suspect it's one of
  the other common shows from thus run, either Sep 30 or Oct 7.  
Nov 6 1966 - Fillmore Auditorium, San Francisco CA
  A recent bootleg CD says this is Grace Slick's first show, but gives it
  this date.  Although JA did play the Fillmore on this date, it is well
  documented that Grace's first show was the day after Signe's last show,
  which would date it Oct 16 1966.  Although no one in the band mentions
  that this is her first show, or at least it wasn't on the tape, Grace's
  singing lacks the confidence of her later shows, and even the confidence
  of the late October Matrix shows, so in my opinion this tape is indeed
  Grace's first show and not Nov 6.  
Feb 3 1967 - Fillmore Auditorium, San Francisco CA
  Tapes with this date appear to be a compilation of earlier shows,
  probably from October or November 1966.  
Feb 8 1967 - Fillmore Auditorium, San Francisco CA
  Again, a date not listed in any Bill Graham or Fillmore documentation.
  I haven't heard this tape, but I suspect it's probably Feb 4 1967.  
Mar 5 1967 - Fillmore Auditorium, San Francisco CA
  Another undocumented Fillmore date.  
Jun 16 or 18 1967 - Monterey Pop Festival
  Their set was Saturday evening, June 17.  
Jun 21 1967 - Fillmore Auditorium, San Francisco
  Yet another mislabeled copy of Toronto 8-5-67.  
Aug 3, 4, 5 1967 - Losers, San Jose CA or Winterland, San Francisco CA
  Although JA did play Losers on these dates in 1966, they were playing
  the O'Keefe Centre in Toronto on these dates in 1967.  The frequently
  mislabeled Aug 5 1967 tape is from the O'Keefe Centre, Toronto, although
  it is often listed as a Fillmore, Winterland, Matrix or Losers show.  
Sep 5 1967 - Studio rehearsals
  Probably the Oct 5 1967 rehearsals.  
Sep 5 1967 - Fillmore or Winterland, San Francisco CA
  Actually Winterland Oct 14 1967.  
Oct 15 1967 - Studio rehearsals
  Also probably the Oct 5 1967 rehearsals.  
Oct 19 1967 - Warfield Theater, San Francisco CA
  Tapes with this date are probably an edit of Oct 14 1967 and June 13 1969.  
Oct 31 1967 - Terrace Ballroom, Salt Lake City UT
  This is an entirely bogus show and date.  JA were in Hollywood recording
  tracks for Baxter's, so it is highly unlikely that they would have
  traveled to Salt Lake City on the same day.  This date is the fabrication
  of a bootleg cd producer, and the bootleg is copied from legimately
  released live cds, including the Fillmore show in the "Loves You" box,
  "Bless Its Pointed Little Head", and Woodstock.
  Also listed as "Langoon Opera House".  
Jan 30 1968 and Feb 1 1968 - Matrix, San Francisco
  Tapes of both dates appear to be a mixture of both shows.  Check the set
  lists.  The set lists given here may not be totally accurate, but are our
  best guess after listening to multiple copies of both dates.  
Sep 15 1968 - Concertgebouw, Amsterdam
  The venue is frequently incorrectly listed as the Paradiso, Amsterdam.
  All reliable documentation, including the announcer on the radio
  broadcast, posters, and people who were there say it was at the
  Concertgebouw.  
Oct 30 1968 - Matrix, San Francisco
  Tapes with this date are a combination of 1-30-68 and 2-1-68.  
Jan 2 1969 - Elysian Park (or Theater), Los Angeles (or Big Sur) CA
Feb 11 1969 - Elysian Park (or Theater), Los Angeles (or Big Sur) CA
  Actually Nov 2 1969.  
Jun 13, 14, 16 1969 - Family Dog at the Great Highway, San Francisco CA
  This run is a confusing mess.  It was broadcast several times on a
  San Francisco station, once as the uncut concert and again as edits
  from more than one show.  Some tapes even include 1967 recordings
  as this date.  Although the setlists may not be completely accurate,
  they are my best attempt to sort them out.  
Jul 9 1969 - Winterland, San Francisco CA
  Another Winterland date with no documentation.  Tapes of this show
  are most likely the Family Dog, Jun 16 1969.  
Aug 2 1969 - Fillmore East, New York NY
  There is no documentation of JA playing the Fillmore East on this date.
  I believe tapes with this date are Oct 4, 1970.  
Sep 1 1969 - Baton Rouge LA
Sep 7 1969 - Family Dog, San Francisco CA
  Tapes of this show dated Sep 1 1969 have been circulating for years.
  However, it was broadcast on the Deadhead Hour in 1996 as the Family Dog
  in San Francisco, Sep 7 1969.  A copy of this tape in the Dead's tape
  vault is dated Sep 7 1969, and Dick Latvala and David Gans both believe
  this is the correct date and venue.  
Oct 31 1969 - Forum, Los Angeles CA
  At least one bootleg album claiming to be this date is really
  Fillmore East, May 7 1970.  Check the set lists.  
Nov 1, 2, 3 1969 - Elysian Park, Chicago IL
  There is no Elysian Park in Chicago, it's in Big Sur, outside of
  Los Angeles.  The only date JA is known to have played Elysian Park
  of these dates is Nov 2.  All others are probably mislabeled Nov 2 tapes.  
Feb 7 1970 - Anaheim CA
  Most likely Anaheim, Jun 7 1970.  
Feb 23 1970 - Auditorium, Dallas TX
  Actually the Winterland, same date.  
May 3, 10, 15 1970 - Fillmore East, New York NY
June 15 1970 - Fillmore East, New York NY
  JA played the Fillmore East only on May 6 and 7, any other May 1970
  dates for the Fillmore East are false.  All tapes with these or other
  incorrect Fillmore East dates are usually May 6 or 7.  
Jul 6 1970 - Anaheim CA
  Most likely Anaheim, Jun 7 1970.  
Dec 31 1970 - Winterland, San Francisco CA
  Tapes with this date are Oct 4 1970.  There is no evidence that JA
  did a New Year's Eve show, it is believed to be just Hot Tuna.

This list began as a compilation of a list posted in January 1995 by the
late Johnny Blasi, and one I started compiling in 1988.  Many errors have
been corrected from the first version, and new sections have been added for
radio and television broadcasts, movie appearances and known tapes.  Any
obvious errors are noted, such as several shows with the same date.  Also,
dates often suffer American/European transpositions, such as 11-2-69 being
interpreted as November 2 or February 11.
